What to anticipate as an enrollee:
After you have been identified as a qualified participant, ViCare will send you a brochure and your physician will provide an enrollment letter.
Next, you will meet with your ViCare guide, a Licensed Clinical Social Worker (LCSW). You and your guide will develop a program of modest lifestyle changes and you will sign a contract agreeing to exert your best efforts.
Finally, you will meet with your ViCare guide face-to-face weekly, anywhere; and utilize the customized ViCare software to achieve your goals step by step.
The ViCare Approach
Building ViCare with the following 5 point approach - we ensure that your stress and conditions are
met with the most comprehensive level of care possible
Kellie meets once-per week with her ViCare Guide.
They work together to determine a care plan, and
Kellie expresses her desire to lose weight.
Kellie now sees her primary care physicians regularly. She is more active and taking less diabetes medication. Both actions have helped Kellie lose weight.
Kellie and her Guide text regularly and meet in person bi-monthly. Kellie’s COPD symptoms have lessened, and she more productive at work and spending time with her kids.
Kellie has had zero ER visits in the past 6 months. She exercises regularly and updates her Guide when medical complications arise.
